The Smile Restoration Project

Each year, we restore one deserving neighbor’s smile — a complete, fixed full-arch restoration provided at no cost, with implants generously donated by our partner BioHorizons. Because sometimes, restoring a smile restores a life.

Share Your Story

Apply for yourself, nominate someone you love, or — if you’re a dentist — refer a patient.

What the Recipient Receives

One selected recipient will receive the following complimentary care, provided at our Southaven office by our doctors:

✦
Comprehensive implant consultation and examination
✦
3D CBCT imaging and diagnostics
✦
Complete treatment planning
✦
Dental implant surgery for one arch
✦
A fixed, full-arch implant restoration (as determined clinically appropriate by the doctor)
✦
Post-treatment follow-up visits during the treatment period
Please note: The care described above is provided at the treating doctor’s discretion and only if the selected recipient is determined to be a suitable clinical candidate following examination. Treatment outside the scope described above is not included, and ongoing annual maintenance, routine hygiene visits, and long-term follow-up care after the treatment period are the recipient’s responsibility. Additional fees may be incurred in individual cases. Dental implants for this program are generously donated by BioHorizons. This program is not a contest of chance; there is no purchase, payment, or obligation required to apply.

Three Ways to Open the Door

Choose the path that fits you. Applications are accepted online only — each takes about ten minutes, and every story is read with care.

Your Information

The person filling out this form. If you're nominating someone else, their details come next.

About the SmileWhat dental challenges is the applicant experiencing? Check all that apply.
In your own words, briefly describe the financial circumstances that have kept treatment out of reach. This stays confidential.
This is the heart of the application. How has tooth loss affected daily life — confidence, health, work, family, faith, relationships? What would restored teeth make possible again? There are no perfect answers. Just be honest. (Aim for 300–1,000 words.) A written story, a description of financial circumstances, and current photos are all required to submit; a short cellphone video, emailed to us, is strongly encouraged.
No file chosen
𝐓𝐡𝐞 𝐕𝐢𝐝𝐞𝐨 (𝐎𝐩𝐭𝐢𝐨𝐧𝐚𝐥)
The strongest applications include one. Using your cellphone, record a short video (1–3 minutes) of the applicant telling their story in their own words, and email it to smilerestoration@adatmj.com with the applicant’s full name in the subject line. Because publicly sharing the recipient’s journey is at the heart of this program, the video helps our team hear the story firsthand – and see that you’re comfortable telling it on camera. Nothing sent to us is ever shared publicly without signed written permission.
Before You Send ItA few things we’re required to confirm — and that protect the applicant.

How the Recipient Is Chosen

This is not a drawing, and no one is chosen by luck. Every application is read by our team and weighed on five things:

Financial need

Treatment is genuinely out of reach without help.

The story

How deeply tooth loss has shaped daily life.

Clinical suitability

Determined by the doctors after examination.

Impact

What restored teeth would make possible again.

Commitment

Willingness to complete all appointments and care.

Finalists contactedNovember 2026

Treatment begins in January 2027, and — with the recipient’s blessing — the journey will be shared through the spring. Every application receives an acknowledgment when it arrives, and every applicant hears from us once the recipient is selected.

The Care Behind the Gift

The awarded treatment is provided by the doctors of Advanced Dental Implant & TMJ Center in Southaven, Mississippi.

Dr. Pradeep Adatrow, DDS, MSD

Dual Board-Certified · Periodontics & Prosthodontics

One of fewer than 100 clinicians in the United States board-certified in both the surgical and restorative specialties of implant dentistry — which means the recipient’s entire treatment, from surgery to final smile, is guided by a specialist certified in every phase of it.

Dr. Jay Patel, DDS, MPH

Board-Certified

Trained at the University of Southern California and the University of Pittsburgh, Dr. Patel shares the practice’s belief that advanced care should change lives — not just smiles.

Questions Families Ask

Is this a random drawing or sweepstakes?
No. There is nothing random about it. Applications are thoughtfully reviewed based on financial need, the applicant’s story, and clinical eligibility. No purchase or payment is ever required to apply.
Can I nominate someone else?
Absolutely — that’s half the heart of this program. Family members, friends, coworkers, neighbors, and dentists are all welcome to nominate someone deserving. Please make sure the person knows and is open to being contacted.
Does everyone qualify?
No. The recipient must demonstrate genuine financial need and must be found clinically suitable for the treatment after an in-person examination by our doctors.
Will the recipient have to pay anything?
The care described in “What the Recipient Receives” is provided at no cost to the recipient. Treatment outside that scope is not included, ongoing annual maintenance and routine follow-up care after the treatment period are the recipient’s responsibility, and additional fees may be incurred in individual cases — anything of that nature is discussed openly and in advance.
What if the selected person isn’t a candidate for implants?
If an examination shows the selected applicant isn’t a clinical candidate for the described treatment, the doctors may recommend an appropriate alternative, or another finalist may be selected. Patient safety always comes first.
Will the recipient’s story be made public?
Yes — sharing the journey publicly is part of the program. Applicants should apply only if they (or the person they’re nominating) are comfortable with their story, photos, and videos being shared. The recipient signs a written media authorization before treatment begins, and nothing is published before that authorization is in place.
I’m a general dentist. What happens to my patient relationship?
Nothing changes. Your office remains the patient’s dental home for routine and ongoing care. We provide the specialty treatment described in the program, keep you informed throughout, and return the patient to your care — with a smile you helped restore.
